(July 1, 2008)--An East Texas dialysis center closed since April after an increase in deaths and life threatening complications will reopen Wednesday.

The state cleared the DaVita facility in Lufkin to resume operations.

A former DaVita nurse has been accused of intentionally injecting two patients with bleach.

Both patients survived.

Kimberly Clark Saenz of Pollok has been charged with two counts of aggravated assault.

A spokesman for the Department of State Health Services told The Lufkin Daily News that the facility will be monitored and will gradually build up the number of patients.

Most details of the center's deficiencies were blacked out in documents obtained by the newspaper through an open records request.

The number of patient deaths under review has not been released.
